GET A VOTER REGISTRATION FORM

• Pick up a voter registration form at your local library, high school, city or township clerk’s office, or public assistance office;
• Request a voter registration form over the phone from your city or township clerk; or
• Download Now.

COMPLETE THE FORM
Don’t forget the following:

• Check “Yes” in the two boxes at the top of the form.
• Give your street address. P.O. Boxes are not allowed.
• Do not forget your Date of Birth.
• You must give either your license number OR the last four digits of your social security number!
• Do not forget to SIGN your registration form in BOTH places!

RETURN THE FORM

• Deliver or Send to City or Township Clerk.
